CHINA TOURIST VISA REQUIREMENTS

Note: All documents must be submitted in A4-size bond paper except for original documents.

Official Accreditations:

Department of Tourism Accredited Travel Agency since 2019.
Allied member of the Philippine Tour Operators Association. (PHILTOA)
Philippine Travel Agencies Association, Inc. (PTAA) member.

Original Passport (Old and New)
Passport size picture (2 pcs, must be taken from photo studio, with formal attire and have white
background)
Travel History (photocopy all passport pages with stamps, visa stickers, and e-visa)
Accomplished Application Form
Original Latest PSA Birth Certificate
Original Latest PSA Marriage Certificate *if married
If your husband/wife is deceased, kindly provide Death Certificate
Annulment/Divorced papers (if applicable)
Photocopy of Valid and/or Government ID (back-to-back)

Original Bank Certificate (ideal amount for current balance is Php 200,000 while the ideal ADB
is Php 100,000)
Note: The Bank Certificate must be addressed to the Chinese Embassy in the Philippines
(1235 Acacia Street, Dasmarinas Village, Makati City)
Original Bank Statement (6 months)

Land Titles
Official Receipt (OR) and Certificate of Registration (CR)
Credit Card (front only)
Latest 3 months billing statement of Credit Card

- Original Certificate of Employment (must be signed by the HR or any supervisor, indicate
  applicant’s position, date hired, compensation, office address, HR landline number, and email
  address)
- Original Approved Leave of Absence (the date in your LOA must correspond to your travel
  date)
- Photocopy of Company ID
- Photocopy of Latest ITR (if no ITR, kindly provide a notarized affidavit explaining why there is
  no ITR)
- Photocopy of PRC Card or IBP Card (if applicable)
- Photocopy of DTI or SEC Permit (if under SEC, must include GIS, Articles of Incorporation, and By-Laws)
- Photocopy of Business Permit or Mayor’s Permit
- Photocopy of BIR Certificate of Registration
- Photocopy of Latest ITR (if no ITR, kindly provide a notarized affidavit explaining why there is no ITR)
- Photocopy of PRC Card or IBP Card (if applicable)
- Notarized Affidavit of Support
- Proof of Funds (please refer to the list of documents for proof of funds)
- Proof of Relationship (Marriage Certificate)

- Original Certificate of Enrollment
- Latest School ID (back-to-back)
- Copy of School Calendar
- Notarized Affidavit of Support

- Retirement Certificate (if retired)
- Certificate of Employment (if under company)
- Contractual Employment
- Client’s bills / invoices / e-mail confirmation / bank transaction references
- Latest ITR (if no ITR, kindly provide a notarized affidavit explaining why there is no ITR)
- PRC Card or IBP Card (if applicable)
- Notarized Affidavit of Support
- Sponsor’s Passport Bio page
- Proof of Relationship (PSA Birth Certificate and/or Marriage Certificate)
- Sponsor’s Proof of Funds (please refer to the list of documents for proof of funds)
